In Hillsborough County, low-income adults face significant barriers to accessing preventive healthcare, including cost, limited provider availability, and lack of care coordination. As a result, this population experiences delayed diagnosis and poor management of chronic conditions such as hypertension, diabetes, and high cholesterol. These gaps lead to increased emergency room utilization and avoidable health complications. A structured, low-cost access point for screenings, early detection, and prescription support is needed to reduce these disparities within a short, measurable timeframe.

Synergy Health Centers (SHC) is an STD Clinic and healthcare equity solution for Tampa-Hillsborough Action Plan, Inc. (THAP)  Our Synergy Health Access and Prevention Programs (SHAPP) serve adults and adolescents in underserved communities who need timely, confidential, and affordable STI screening, treatment linkage, and prevention care. Many Hillsborough County residents from underserved communities experience delays in testing, missed follow-up appointments, limited awareness of covered preventive services, untreated infections, stigma-related avoidance of care, and low utilization of HIV prevention options. These barriers increase the likelihood of undiagnosed sexually transmitted infections (STIs), continued transmission, and preventable long-term health complications.

SHAPP addresses these barriers through coordinated screening, treatment navigation, prevention counseling, insurance benefit guidance, and follow-up care management designed to improve testing rates, treatment completion, prevention uptake, and continuity of sexual healthcare.
